MedStaff Local provides two types of dental plans for our healthcare professionals:

• UHC Dental Plan Comparison (PDF file, 32 KB)
• Dental Enrollment Form (PDF file, 108 KB)

You can click on either plan above to get specific information regarding Monthly Payroll Deduction, Deductibles, Services Covered, Eligibility, etc. Once you've decided on the plan you desire, please download the enrollment form and complete it.
