ABOUT PER DIEM - benefits - Life Insurance

Medstaff is pleased to offer FREE Life Insurance to all full-time employees. All employees who work a minimum of thirty (30) hours per week are eligible for this benefit.

Life Disability Provision*	Premium Waiver to Normal Retirement Age if Disabled Prior to 65
Living Benefit Option (Accelerated Death Benefit)*	12 months Life Expectancy, 80% of benefit to Max. $20,000
Conversion*	Included
Accidental Death & Dismemberment (AD&D)	Flat $20,000
Employee Contribution	Non-contributory

Please note: If employee participating in healthcare coverage fails to maintain 90 hours for the previous one (1) calendar month period, employee will be dropped from coverage on last date worked that month and appropriate COBRA paperwork will be mailed to employee.
